Quite often I or others on my server will just randomly get this as a popup in their screen that you have to click "Okay" to get rid of. what's up with that?
 
I don't think so, I would have thought maybe hackers were trying to spawn weapons that don't belong but I actually thing is the custom loot table.

Maybe I am spawning something through the custom loot table it doesn't like?

lol you answered your own question

But yes some weapons get "errors" because rocket messed with cfgweapons in an attempt to blacklist weapons
