Respawning same place/Gear bug

Ive had complaints from players saying they are logging out before restart then logging in, when they log in they are at the coast but have their gear, they havnt died they say.

Some people say that when they die they are re spawning where they logged in and have the same gear, no idea where this bug came from,

The map is on taviana and im running multiple scripts like lift/tow, admin tools, ect,ect.
Im running a default server.pbo as i havn't been required to modify it

I had a similar problem with my new setup sever, I respawned at the same place I died unconscious and died again.. in a loop. I dropped my database and refreshed it, that fixed my problem.
 
I am having a issue like this, but it is only me not the players on my server. I spawn 22000 m away from the map and I have no sound, no food/health bar. I hope someone will reply how to fix this.
