I got that message before and after researching for awhile finally found out it was because my pbo wasn't packed correctly. It might not work for you but I fixed it by downloading a stock PBO, unpacking it and then copying everything from my custom mission pack into the stock one, and repacking it.

Can only speak from my own experience but I've found that this problem happens most of the time after a restart when a player clicks 'ok' to load in before the server timeout countdown has completed.

Ordered last Wednesday, hadn't heard anything from them about it at all going into the long weekend but figured they were just enjoying themselves over the holiday. Still have heard anything and now it seems like the site is down again. :(

You mentioned that you had to modify to the sever_cleanup.fsm and server_monitor.sqf files, but did you make sure you modified the init.sqf too? Don't forget you still need to chance "enableradio = false " to //enableRadio false;
